An eldercare assessment is
a systematic survey of the "situation"
surrounding an elder and his or her caregivers. Together
the elder, the family and an eldercare professional take
a really in-depth look at the elder's medical and
physical challenges, the suitability of the elder's
residence, the financial resources available, and the
support systems that are in place, or that could be
accessed.
Along with these
descriptive "facts" the
emotional factors - everyone's "wants" - are
just as important.
A thorough
assessment will help you identify what's working and show you where
you might have options you didn't know about. At the end
you'll have a "roadmap" that shows you where you are
now and suggested routes and resources for moving ahead through the eldercare
maze.
An eldercare
assessment takes place wherever your elder is living (or in the
hospital, if necessary). It usually requires about two to four hours and the exchange of lots of detailed
information.
Within a few days
of the assessment you will receive a complete written summary
called a "Care Plan."
